Laboratory Services Guide

The laboratory services guide provides detailed information about each test and the requirement for that type of testing (sample size, sample collection, stability and refrigeration requirements). Updates are available in real time on the Brown University Health Intranet, however a Brown University Health login is required. Updates made to the online lab guide may not immediately be present in PDF form. Please note the publication date, located in the lower right corner of the document.

Home Draw Requisition

All orders must be submitted on a Brown University Health Laboratories home draw requisition and signed by an authorized health care provider. Orders should be faxed to 401-793-4741. No verbal orders can be accepted. Incomplete requisitions will delay the scheduling of a home draw.
