The Transition Consultation Clinic is designed to assist families in managing the complex passage from pediatric to adult care. The clinic is held once a month in the Children's Neurodevelopment Center.

Our clinic does not replace your primary or specialty care providers, but rather assists you in making the leap from a pediatric to an adult health care environment.
Our multidisciplinary team will help you and your family create a transition plan based on your personal needs and goals. We provide education and support for the necessary steps to transition to adult care services.
