Our care management team includes pharmacists, registered nurses, and care navigators who provide guidance and support to patients managing complex medical conditions, navigating recent transitions of care, or undergoing treatments that require additional assistance. The team also works with patients to educate them about chronic conditions, identify and address barriers to care, and develop patient-centered care plans. The goal of this program is to help keep patients healthy at home and avoid unnecessary hospitalizations.

Our nurse care managers and practice nurses assist patients in managing their conditions, coordinating care between visits with primary care clinicians, and ensuring that treatment plans are followed. We also offer educational materials designed to address the needs of patients with complex conditions.
Our clinical pharmacy team consists of clinical pharmacists, pharmacy navigators and pharmacy technicians to provide clinical pharmacy services and medication support to our patients.
- Pharmacy technicians and navigators support medication refills, cost or coverage issues.
- Clinical pharmacists meet with patients at our practices, over the phone, or through telemedicine, for comprehensive medication reviews. In collaboration with our clinicians, we ensure our patients receive the most effective therapies for chronic conditions such as diabetes, high blood pressure, lung disease or heart disease.

Through this program, Brown Health Medical Group Primary Care provides comprehensive evaluation and discussion on health preferences to help you reach your goals.
